  • The planking or support gave out. This is usually caused by failure of scaffold parts, overloading, defective scaffolds, scaffolding was poorly inspected—or not at all.
  • The worker slipped
  • The worker was struck by a falling object

OSHA has strict standards in place that, if followed, prevent these types of scaffold accidents from even occurring. If you suspect OSHA standards are being violated on your construction site, do not be afraid to report this to OSHA.
